  than restoring physical function\; it depends on coordinated\, person-cen
 tered care that promotes meaningful participation in daily life. Healthcar
 e professionals must understand how the principles of neuroplasticity can 
 be applied through purposeful activities while partnering with individuals
  and their families to establish goals that reflect what matters most to t
 he person served. This session addresses the need for clinicians to streng
 then collaborative practice between occupational therapy and physical ther
 apy\, incorporate shared decision-making into rehabilitation planning\, an
 d apply evidence-informed strategies that support individualized recovery\
 , functional independence\, and return to meaningful roles and activities.

 understand how meaningful activities can promote recovery and participatio
