DESCRIPTION

Integrated, whole-person care is a growing area of focus among providers in Iowa, but there are still gaps in knowledge and competence. This lunch and learn series will address some key areas of integrated care in medical and behavioral health settings. 

LEARNING OBJECTIVES

  • Examine the Social Ecological Model and how it can support whole-person care.
  • Define Social Drivers of Health (SDoH) to contextualize leveraging patient data for care planning and treatment.
  • Increase knowledge surrounding the context for providing equitable, responsive care in integrated care settings.
