PURPOSE

This online activity aims to provide cardiovascular health-related information and resources, specifically about policy and systems changes in healthcare systems and clinics for diagnosing and managing hypertension. Participants will learn of specific intervention protocols and their implementation to improve the management and control of hypertension.

LEARNING OBJECTIVES

1. Define the core principles of team-based care (TBC). 
2. Identify the best methods of communication between TBC team members.  
3. Define procedures that enable the engagement of TBC team members outside the health systems, such as local pharmacists, community health workers, and more.   
4. Describe skills needed to include and engage patients with the team-based care team.
